First Visit
Your initial appointment will consist of a comprehensive examination, a baseline series of x-rays, and a periodontal charting of your gums to assess your gum health. The doctor will review any treatment needed. After this appointment, we will make an appointment for your first cleaning.
The following information is needed before your first appointment:
- Any x-rays taken by a previous dentist, if applicable.
- Fill out the Health History Form that is e-mailed or texted to you after you make your appointment.
- If you have dental insurance, bring your insurance card to your appointment.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your previous dentist has taken x-rays, you may request that they forward them to our office before your appointment. Otherwise, x-rays will be taken at your first appointment.
